This Parental Consent Agreement enables parents or legal guardians to authorise a young person aged 14–17 to use Link Golf UK golf courses and/or driving ranges without direct adult supervision.

By completing this form, you confirm that you have read and understood the conditions of use, that your child understands and agrees to follow all safety rules, course etiquette and staff instructions, and that you accept the responsibilities outlined within this agreement.

This consent applies only to the Link Golf UK venues specified and remains valid until withdrawn in writing or replaced by an updated consent form.
